Montana is one of the thinnest states in the union, but we're still not one of the healthiest.

In fact, 60 percent of adults in Montana report having at least one chronic condition. Most of these conditions are brought on by obesity, lack of physical activity and tobacco use.

Regardless of the cause, the conditions and complications associated with them are real and it's often tough to find treatment for them. The Department of Public Health and Human Services has simplified this with their new Community Health Program Guide complete with an interactive map.

If you, or someone you know, is struggling with a chronic condition, like diabetes or depression, have them take a look at the guide. For most, it is encouraging to realize the amount of help that is available to treat or prevent these conditions.
